Anointing of the Sick

The Sacrament is normally given to persons who are very ill or approaching death; or to those who are aging or approaching a serious surgery. It is nice to be able to gather family and friends together to celebrate the sacrament.


The person, a friend or a family member should call the parish office  (541-942-3420) to make an appointment for an anointing.  With advance notice, anointing may be held after each weekend Mass.


Baptism and Confirmation Preparation:

Contact Betty Krumlauf  via the parish office (541-942-3420) to schedule an initial meeting for the required parental and godparent baptismal preparation.


The sacrament of Baptism is celebrated within the weekend masses at OLPH or St. Philip Benizi. Advanced scheduling and confirmation with priest or deacon is required.


Download the Request for Baptism form (Formulario de solicitud de bautismo), print out and fill out as much as possible before the first baptismal preparation meeting.


Marriage Preparation

Those wishing to be married at OLPH or St.. Philip Benizi should contact Deacon Kenneth Boone via the Parish Office (541-942-3420) no less than six months prior to their anticipated wedding date to schedule an initial meeting. Please do not finalize any arrangements until after meeting and confirmation with deacon.
